We're excited about your interest in Shore Staffing.  There are competencies and mandatory forms that must be completed prior to employment.  Please review the following, print them, complete and fax to us after you have completed your application on line.  (APPLY ON LINE)

As you know, hospitals and other health care facilities require a variety of records and competencies.  These vary from facility to facility, but some items are mandatory for everyone.  We appreciate your completion of this process.  Once this is complete, you will be ready to go.

TO GET STARTED WITH SHORE STAFFING, please complete the following:

1.     Complete the application on line.  You must be sure to complete in full each blank so we will have the appropriate information to consider you for employment.

• LICENSURE/CERTIFICATION:  We will need a current signed copy of your license or certification for your discipline.  If you are in Maryland, we will obtain a copy of your license upon receipt of your license number. 
• HEALTHCARE PROVIDER CARD (BLS, NRP, ACLS, PALS)
• ID:  Copy of driver's license and copy of social security card
• Copy of your PPD (required annually)
• Copy of Hepatitis B - documentation of a declination, completed series, booster or a positive titer.
• MMR/Rubeola - documentation of two (2) MMRs, two (2) vaccines or a positive titer.
• Varicella - documentation of a written history, MD statement, vaccine, or positive titer.

2.    Complete the following forms prior to your interview (required for employment):
        HIPAA (with direct deposit information and background check authorization)        
        MANDATORY EDUCATION COMPETENCY
        PHYSICAL (within last 6 months)        
        EMPLOYMENT STANDARDS
        TB Questionairre
        MEDICAL QUESTIONAIRRE
        W-4
        Proof of eligibility to work in US (I-9)
        Three Professional references (download these copies, and have at least 3 references fill out a form and fax or email back to us.  These must be professional (your supervisor or other supervisors where you have worked)

3.   Complete your skills checklist based on your specialty area or position held.  
        Work Experience Checklist - complete this in detail
        Med/Surg skills checklist for LPN or RN
        Nursing Assistant checklist
        ICU skills checklist for RN
        ER skills checklist for RN
        Telemetry skills checklist for RN
        LTC skills checklist for LPN or RN

4.    Nurses must complete competency tests.  
        Medical Surgical Med Test
        Critical Care (for ICU or ED) Med Test
