If you are a nurse or other healthcare professional, please also provide the following:

1. State licensure information (a copy of your license if your state provides one)
2. Copies of current BLS (front & back signed) and other certifications (ACLS, NRP, PALS)
3. Physical within the last 12 months (updated annually)
4. Hepatitis B documentation, copy of declination, completed series or a positive titer
5. Mantoux TB Screen current within 12 months or Chest X-Ray within 1 year
6. MMR/Rubeola - Documentation of 2 MMRs, 2 Vaccines or a positive titer
7. Proof of immunity to Varicella (positive titer or varivax inoculation)
8. Tetanus within 10 years or signed declination
9. Facility update list...Please provide a list of all facilities you have worked in (either through agency or employed).
